Dosage and method of administration
The dosage depends on the age, weight and renal function of the patient and the severity of the infection.
Adults and children over 14 years:
Intramuscular or intravenous :
1 to 3 g/day divided into 4 IM or IV administrations.
In case of severe infections (osteomyelitis, endocarditis): up to 8 g/day in 4 rapid infusions (20-30 min). No infusion should exceed 2 g.
Haemolytic streptococcal A, B infections require a minimum of 10 days of treatment.
For surgical prophylaxis: 2 g IV (bolus or infusion) at induction of anaesthesia. Repeat every 6 hours for 24 hours in orthopaedic or vascular surgery and for 48 hours in coronary or cardiac surgery.
Children under 14 years:
25-50mg/kg/day to be administered IM or IV and divided into 3 or 4 administrations.
If necessary, the dosage may be increased to 100 mg/kg/day by parenteral administration divided into three or four administrations, but no injection may exceed 33 mg/kg at a time.
Children 10 to 14 years of age usually receive a daily dose of 1.5 g to 2 g in 3 or 4 divided doses.
Children aged 6 to 10 years usually receive a daily dose of 0.7g to 1.5g in 3 or 4 divided doses.
Dose adjustment may be necessary in premature, newborn and breast-fed babies.
In renal failure:
If creatinine clearance falls below 10 ml/min, the recommended dosage is 1 g every 8 to 12 hours (in the anuric patient, the dosage is 1 g every 12 hours).
Dialysis should not be accompanied by an additional dose.
